We respect your privacy and is committed to protecting it through this Privacy Policy. Our privacy commitment is based on the ten principles contained in the Health Information Act. (HIA).
It provides our clients/patients with the right to access their own health information and to request corrections. It protects the privacy and confidentiality of health information and enables health information to be accessed and shared to provide health services with consent.
I authorize the clinic and its associated health professionals to collect my personal and medical information for treatment purposes ONLY. In addition, I authorize the clinic and its associated health professionals to communicate with my family doctor and/or referring doctor as deemed necessary for my beneficial treatment. I also understand that my personal and medical information is confidential and will only be disclosed to third parties with my permission.
Cancellation Policy
A credit card will be required to book all appointments. This credit card will not be charged for your services including pre-payment of IV Drips, this credit card will be charged $75 for no-show appointments or cancellations made with less than 24 hours’ notice. As such we require 24 hours’ notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours’ notice, or miss their appointment, will be charged a cancellation fee to the card on file.
